Athletic Physicals

Annual physicals are required for ALL athletes per UIL. CH Athletics will be providing an opportunity for students to have their athletic physical done at school on Friday, April 28^th for $20. These onsite physicals will be offered to all incoming 7^th – 12^th CH athletes. Students do NOT need to sign up, just bring the form below with the parent section completed and $20 cash or check. Coaches will also be sending this form home with athletes prior to this day. #CHBlueDevilNation
